The Medical Screen
Customers will be screened at the point of sale using a Google Form Survey Web page, a version of the Canadian Physical Activity Readiness Questionnaire (PAR-Q) Customers will be enrolled in the Club membership as you do now.
Automatic Alerts to the Doctor
Once the PAR - Q is filled out, it will land onto the Health Survey Google Sheet system we provide to monitor the progress of the new customer and to provide a reference point for all stakeholders of the programme. In the event that any risk factors are identified on the screening form, the members’ record will be registered as having a POSITIVE PAR -Q result and the Medical practitioner will be notified automatically by the system that the customer requires a consultation. As a club you don’t have to do anything other than load the system with the PAR-Q. It all happens using the magic of the web and the technology!
Automatic notification to the fitness team and voila!
The Doctor receives automatic notification from the system and can LOOK in to the Google Sheet and see all of the contact details of the member and their PAR-Q results. The Doctor will then carry out the telephone consultation and the next action will be one or more of the following:
The customer will then receive a refund of any programme payments made. Once the consultation has taken place an automated email alerts the fitness team that the Client is ready to have their induction and the Doctors recommendations are ready for the fitness team to view. Finally, the Doctor sends an email to the member with a copy letter for the members own GP to make them aware of the intervention.
